Description

This course is designed for ship and shore staff of Bay Ferries Limited (BFL) and Northumberland Ferries Limited (NFL) involved in incident investigation on board or ashore. Utilizing a case study approach, the course focuses on techniques for gathering complete, accurate and objective incident data, establishing root causes, reporting findings and determining corrective action’s). The course is designed for both ship and shore staff who require a better understanding of the principles of incident/accident investigation and root cause analysis. Participants learn how to uncover the why, what, who, when and how of an incident, and how to analyze incident information to prevent future injuries, property damage, and/or financial losses. Learning Outcomes

By the end of the course, participants will be able to:
  • Explain the reasons for carrying out an investigation
  • Understand how to collect evidence
  • Describe the causes of incidents and accidents
  • Understand the significance of the human element in incidents
  • Analyze direct causes and root causes (including control management failures)
  • Create an event map
  • Develop corrective actions post event
  • Describe the process for follow-up
